2025 1095 - C DISTRIBUTION
Current Active Employees:
As part of recent changes to federal law, we are no longer required to automatically mail paper copies of your annual health coverage tax form (Form 1095-C) unless you request one. This form provides information about the health coverage offered by Sumter School District and is a record you should keep for your files, though you generally do not need to submit it with your federal tax return.
If you would like to receive a paper copy of your Form 1095-C, you must submit a request.
To request your paper copy, please contact us using one of the methods below:

Upon receiving your request, we will mail your form within 30 days of your request. Please ensure we have your current mailing address on file.
This notice will remain available on Sumter School District’s website through October 15, 2026.

If you request your 1095 C by mail or email, Include your First and Last Name, Employee ID number or last four of your social security number, and mailing address. If your mailing address does not match what is in our records, you will be required to contact Human Resources to change your address).
Please note, employees that are no longer employed with Sumter School District will receive a paper copy of their Form 1095-C via mail.
